circumstances should the animal be housed outdoors.

______I agree under no circumstance will this animal be bred, resold or rehomed. If the adoptee is
unable to provide or care for the animal any longer, the animal must be returned to the rescue.

______ I agree to provide yearly vet visits, vaccinations and follow any regional animal legislation
applicable to the area in which the animal is housed. I agree and understand the financial responsibility of
owning the above animal.
